Job Purpose:
The Care Coordinator will be responsible for coordinating and scheduling the delivery of domiciliary care services to clients. They will work closely with the Registered Manager, care staff, and clients to ensure that the services are delivered in a safe, effective, and efficient manner.
Key Responsibilities:
· Coordinate and schedule the delivery of domiciliary care services to clients, taking into account their individual needs and preferences.
· Ensure that the care staff have the necessary training and support to deliver high-quality care to clients, and provide regular supervision and feedback.
· Liaise with the Registered Manager and other members of the care team to ensure that clients receive coordinated and integrated care.
· Maintain accurate records of client care plans and service delivery, and ensure that all documentation is completed in a timely and accurate manner.
· Monitor the quality of the care services, and identify areas for improvement and development.
· Manage and oversee the implementation of technology and digital solutions to enhance service delivery.
· Manage and maintain relationships with clients, their families, and other stakeholders, and respond to feedback and concerns in a timely and appropriate manner.
· Participate in the recruitment and selection of care staff, and ensure that they have the necessary qualifications and experience to deliver high-quality care.
· Develop and maintain effective working relationships with other healthcare providers and local authorities to promote the services and increase referrals.
· Keep up-to-date with developments in domiciliary care, and ensure that the services are delivered in accordance with best practice and evidence-based guidelines.
Qualifications and Experience:
· Relevant qualifications in health or social care, or a related field.
· Proven experience in coordinating and scheduling domiciliary care services, including experience in managing staff and schedules.
· Excellent knowledge of regulatory requirements and standards relating to domiciliary care.
· Strong communication and interpersonal skills, with the ability to build effective working relationships with clients, families, and other stakeholders.
· Strong organizational and time management skills, with the ability to manage multiple priorities and deadlines.
· Knowledge of digital solutions and technology in domiciliary care services.
· This is a full-time role, with flexible working hours, including evenings and weekends, as required. The successful candidate will be subject to a Disclosure Scotland check.

Key Responsibilities:
· Provide personal care and support to clients in their own homes, in accordance with their individual care plans.
· Assist clients with activities of daily living, including bathing, dressing, and grooming.
· Administer medication and monitor clients' health and well-being, and report any concerns or changes to the Registered Manager or Care Coordinator.
· Assist with meal preparation and household tasks, and support clients to maintain their independence and dignity.
· Maintain accurate records of service delivery and client care plans, and ensure that all documentation is completed in a timely and accurate manner.
· Participate in training and development activities, and maintain up-to-date knowledge of best practice and evidence-based guidelines.
· Maintain effective communication with clients, their families, and other members of the care team, and report any feedback or concerns to the Care Coordinator or Registered Manager.
· Respect and maintain the privacy and dignity of clients, and promote their rights and choices.
· Follow all policies and procedures relating to health and safety, infection control, and safeguarding.
· Maintain confidentiality at all times, in accordance with legal and regulatory requirements.
Qualifications and Experience:
